You will be able to download and print your Certificate of Attendance upon completion of the online evaluation questionnaire form.

Certificate of Attendance

Follow these 3 steps to obtain your certificate of attendance:

  1. Login with your ESMO username and password
  2. Complete the evaluation questionnaire and click submit
  3. The system will download your certificate
